To celebrate the O-Town Fan Page's 2 year anniversary and Ashley's 21st birthday we are having an extreme contest that focuses on the #1 O-Town fan. Only 10 fans will be selected out of thousands of entrants to be included in this contest. The form below must be filled out completely by Monday, July 22nd by 5:00P.M. CENTRAL TIME. No forms will be excepted after this time and date.

The OTFP Team will pick 10 fans to be involved in this contest based on their completed forms. (The 10 will be chosen based on their answers to the questions.) From there on the official contest will begin. Each week we will ask the 10 contestants a question and based on their answers you, the fans, will be able to vote off the contestant with the least liked answer to represent the #1 fan. Each week 2 contestants will be voted off except for the last week.

*You, the OTFP visitors, will be the ones to judge who deserves to be "The Last Fan Standing."*

Monday, July 22nd at 5PM CENTRAL STANDARD TIME, is the deadline! Soon after that, the 10 contestants will be selected and e-mailed with more details about the contest including the rules. Finalists will be posted by Tuesday.

WHAT YOU WIN
We always give out an awesome prize pack so here is the contents of this one! If you are the Last Fan Standing, you will receive:
1. CD of your choice
2. A video tape of over an hour of O-Town footage including footage from their recent "O2" Sneak Preview Tour.
3. A "real" picture of your fave O-Town member.
4. Other small goodies to be announced later.

We ask that if you choose to be a part of this contest that you fill out this form completely otherwise you will be denied!
